Coverage caveats

Extraction is regex-driven. Claims that name a bar but use novel phrasing (e.g. paraphrased or interleaved with prose) will be missed. A verified P4 run will surface these as "silent gaps" when we look at cards that produced zero claims.
Cards without a MIDI paragraph (prose-only tier cards) contribute zero claims. That's honest, not a bug: only claims tied to specific bars go to P4.
Vocal sequence claims use pitch classes only. If P4 wants to check exact octaves, the ledger already carries them under values.octaves.
